Please arrive 10 minutes early to complete your New Patient form, or save time by downloading and filling it out beforehand using the link below.
Initial Examination
Your dentist will begin by reviewing your medical and dental history and completing a comprehensive dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, and any required X-rays.
Treatment Plan
Based on your history and examination, your dentist will develop a dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.
Insurance & Payment
At North Shore Smile Dentistry, we'll help you navigate our options for payment and insurance so you can focus on achieving your dental goals.
Insurance Claims
Our administrative team is happy to assist you in understanding your insurance plan better. For existing patients, we can direct bill your insurance company.
Payment Methods
The full treatment cost (or the patient portion if direct-billing to your insurance) must be paid at the time of your appointment. We accept payment by cash, cheque, debit or most major credit cards.
